Provident Fund Statement

An employee provident fund is a fund established to provide pension plans to employees. The purpose of the fund is to provide financial assistance to employees over a certain age. H. After retirement or if the employee is unable (temporarily or permanently) to work.

EPF Form 5

Form 5 is filed by the employer to submit details of all new employees qualifying for membership to the Employees’ Provident Fund (EPF), Employee Pension Scheme (EPS) and Employees’ Deposit Linked Insurance Scheme (EDLIS) for the first time during every month. However, if no new employee has joined, ‘Nil’ will have to be mentioned in the form. It is to be filed by 15th of the following month.

Return of employees who are entitled and required to become members of the Employees’ Provident Fund (EPF) and Employee Pension Scheme (EPS). (Within 15 days of coverage)

EPF Form 6A

This form provides annual contributions of each member of the establishment. It is a necessary form for compiling the yearly Provident Fund statement of a subscriber. It is to be filed by the employer before 30th April.

EPF Form 3A

A form showing month wise recoveries towards EPF and EPS for one financial year of one member. Also called as the Member’s Annual Contribution Card. It is to be filed by the employer before 30th April.

EPF Form 12A

Form 12A is a Consolidated Statement showing amounts recovered from employees and contributions remitted relating to Employees’ Provident Fund (EPF), Employee Pension Scheme (EPS) and Employees’ Deposit Linked Insurance Scheme (EDLIS). It is to be filed by the employer before 25th of the month following the month to which the dues relate.

EPF Form 10

Form 10 is filed by the employer to submit details of all employees leaving the organization. The date and reason for leaving service will also have to be mentioned.

It is filed along with the other forms and ECR. If no new employee has quit the organization, ‘Nil’ will have to be mentioned on the form. It is to be filed by 15th of the following month.
